Universal intake fields that every studio needs.

Keep the core intake form short and focused. Clients are more likely to complete it when it takes less than five minutes.

  • Full name, email, and phone.
  • Preferred contact method.
  • Emergency contact for services with higher risk.
  • Allergies, sensitivities, and medications that impact the service.

Service-specific questions that matter.

Use targeted questions based on the service. Too many questions can feel invasive, but the right ones improve outcomes and reduce risk.

  • Skin services: active ingredients, recent treatments, pregnancy status.
  • Lash services: eye sensitivities, contacts, previous reactions.
  • Massage: injuries, pressure preferences, contraindications.

Consent and release language to include.

Consent builds trust and protects your studio. Keep it clear and easy to understand, and always include a checkbox that the client must actively select.

  • Consent to service and acknowledgment of risks.
  • Permission for treatment photography if relevant.
  • Agreement to policies and cancellation terms.

Privacy and data handling basics.

Let clients know how you store and use their information. This can be one short paragraph on the form or linked to your privacy policy.

  • Only collect data you actually use.
  • Store forms securely and limit access to staff.
  • Explain how clients can request updates or deletion.

Questions to avoid or keep optional.

Long forms reduce completion rates and slow down booking. Keep optional questions optional and avoid collecting unrelated data.

  • Unnecessary demographic questions.
  • Detailed medical history unrelated to the service.
  • Long surveys before the first visit.
